Financial Information

Dr. Armitage requires full payment at the time of service. This policy allows us to keep our fees for services as low as possible. We appreciate your cooperation.

We will be glad to file your insurance claim for you. However, we ask that you are prepared to pay deductibles and co-payment estimates at the time of service. (Please understand that all insurance co-payments are an estimate only. Your actual payment may be more or less than quoted to you. You are financially responsible to us for all services rendered)

Any fees incurred in collection will be added to the patient's account. This includes NSF and return checks, placement with a collection agency or small claims court filing. If you should encounter a problem, please communicate with us as soon as possible so that we will be able to work with you.

In the event of services that are incurred by a minor patient, both parents are financially responsible regardless of the divorce decree.

For our patients without insurance we offer an in-house discount plan that covers 2 annual checkups, 1 emergency exam and 10% off any other treatment for a low monthly payment.  For more information please call the office or go to www.kleer.com by clicking on the box on the right.
